If you are elibible for a flu vaccination, please contact the surgery now to book your appointment.

Our main flu clinics are on Saturdays 21st & 28th October, but other times are available;

The eligibility criteria are as follows:

  • are aged 65 and over
  • have a chronic heart condition
  • have a chronic chest complaint, including asthma which requires regular medication
  • have chronic kidney disease
  • have chronic  liver disease
  • have a chronic neurological condition
  • have diabetes
  • have lowered immunity due to disease or treatment such as steroid medication or cancer treatment
  • have no spleen, or your spleen doesn’t work fully
  • are a morbidly obese adult (BMI of 40 or above)
  • are pregnant
  • are living in long-term residential or nursing home

Pre-registration with the practice

PreRegistrationIf you wish to pre-register click on the link below to open the form. When you have completed all of the details, click on the "Send" button to mail your form to us. Please visit the surgery as soon as possible after sending the form as you will need to sign it to confirm that the details are correct. You will also be required to make an appointment with one of our Health care Assistants or Practice Nurse to have a New patient health check.

When you register you will also be asked to fill out a medical questionnaire. This is because it can take a considerable time for us to receive your medical records. There is an online version of this file too, which you may fill out and send to us. When you come to the surgery you will be asked to sign this form to confirm that the details are correct.

Note that by sending the form you will be transmitting information about your self across the Internet and although every effort is made to keep this information secure, no guarantee can be offered in this respect.


Alternatively you may print off a registration form, fill it out and bring it in with you on your first visit to the practice.
